Course Description
Medical record documentation is the source used to accurately code encounters provided by physician and non-physician providers. The provider of service is responsible for ensuring the accuracy and the quality of the medical record. The documentation from a provider evaluates and coordinates patient care, tracks and monitors disease processes, provides insight into quality of care and provides information for the purpose of claim submission and payment for services rendered.
