Chapter Seven Part 1 Reading Assignment
Surgery is defined as the art, practice, or work of treating diseases, injuries, or deformities by manual or operative procedures. Procedures may be invasive or non-invasive (i.e. closed fracture care) and may include instrumental applications to treat such cases. Surgeries are further classified as minor or major procedures. Minor procedure examples include ear wax removal, diagnostic scope or joint injection. Major procedure examples include knee replacement or spinal fusion.

Answer the following questions related to surgery coding.
1. Three lacerations of the dermis and subcutaneous tissues were repaired followed by a layered closure. These lacerations were on the patients arm. They individually measured 2.1cm, 1.9 cm, and 3.0 cm. Code the repair.
2. A patient is brought to the ED after a motor vehicle accident. She has a proximal open tibial fracture. The physician performs extensive debridement of gravel, glass and other matter, down to and including part of the muscle at the site of the fracture. The patient is then transferred to another facility for definitive fracture treatment. Code the procedure(s).
3. A patient with multiple basal cell carcinoma (CA) lesions of the arms presented for removal. A malignant lesion with an excised diameter of 1.5 cm is excised from the left arm, and another malignant lesion with an excised diameter of 2.0 cm is excised from the right arm. Both were repaired with primary simple suture closure. Code the appropriate CPT procedure code(s).
4. A 45 year-old patient was brought to the ambulatory surgery area for treatment of multiple plantar warts. The physician used a laser on a total of 30 warts. Code the appropriate CPT code(s) for this service.
5. A child is seen at 9AM for a right nursemaids elbow by Dr. Mickey Mouse. He performed a manipulation to treat the subluxation. The child returns to Dr. Mouse at 4PM the same day for a re-subluxation of the right elbow which required a second manipulation. Code for the procedure (s) performed.
6. A patient undergoes arthroscopy of the right shoulder with extensive debridement and subacromial decompression and resection of the acromioclavicular joint. An arthroscopic partial rotator cuff repair was performed for a chronic rotator cuff tear. A bursectomy was performed as well.
7. A patient undergoes a right proximal phalanx osteotomy and distal first metatarsal osteotomy to correct a severe hallux valgus.
8. An ED physician applies a right short arm splint that allows movement to the patient, who will see the orthopedic surgeon for definitive treatment in a few days, after swelling has decreased.
